United Glass Tech, Chemglass (they are the shiznit when it comes to custom stuff... very very fast quotes and responses), and just about any other major glassblowing company out there.

Look For Neon sign makers. they're skilled enough to do it.
Because its an uncommon skilled craft someoene who does
glassware repair or restoration , can set any price and get
it. Shop around.

A neon signmaker needs a garage, a butane torch and a vacuum pump. A glassblower needs about a 1/4 M worth of outfitting. And a whole lot more skill .. say ten years of apprenticeship. I have a friend that blows lab glass and it was a family trade. Even for basic repairs one needs an annealing oven and signmakers don't have those. The glasses are completely different too. Sign glass is soft and thin.. you can almost bend it with a blowdryer. Lurker, are those guys in TX or OK?

I like Precision Glassblowing. They a are fast and very reasonably priced smaller shop who actually likes custom work and oddball requests.
