Local gas suppliers seem to charge exorbitant prices to refill CO2 cylinders. It seems that it would make more sense to buy 5lb of pellets from the
local supplier, pour into the neck of a DOT-approved 5lb cylinder and let nature take its course. What am I missing ?

I think that depends on the required purity. Typically, you'd need to purge the cylinder with an excess of CO2 to flush water vapor and
other gases out of the cylinder. I've dispensed propane for years, and this is done for propane tanks to prevent the buildup of ice in the valve.
If you don't need a lot of CO2, you could always keep a carbonate on hand, and just make the gas as needed.

Check out CO2 fire extinguishers, I've heard of people getting them refilled for a competitive price.

just get a cheap one from kegland or a beer fittings supplier.

Check out the places that refill CO2 fire extinguishers, they may have an expired one laying around. I had one given to me just because I asked if
they had any that someone had not claimed after refilling. Turned out it had expired after filling, no claimant, so he just gave it to me.
