Metabolites (from medications) turning them back into active ingredient?

I came across a site listing chems for sale and one stood out b/c it is a controlled substance, but what they were listing was the metabolite. I compared the metabolite and it looks almost identical except it has an extra OH bond on it (excuse me I don't know the terminology when it comes to looking at molecule structures - I need a good course/site on this...)

I've seen a lot of synthesis where people remove various elements of a compound and was wondering if it is possible to extract the metabolites from "excretions" and then do a synth to turn them back into the active ingredient?

Sometimes you don't need to do any "work" on the metabolites.
